(Little Chute, WI) — Affinity Health System and Ministry Health Care are pleased to welcome Courtney Forte, MD, to Affinity Medical Group’s family medicine department. Dr. Forte sees patients at 200 Patriot Drive in Little Chute.

A board-certified family medicine physician, Dr. Forte provides acute, chronic and preventive ambulatory care to the entire family. He has a special interest in sports medicine and infectious diseases. Dr. Forte’s goal is to make sure his patients’ concerns are being addressed. He wants his patients to feel comfortable and confident in their care plans.

Dr. Forte received his Bachelor of Science degree in biology from Berry College in Rome, Georgia. He earned a medical degree from Ross University School of Medicine in Portsmouth, Dominica. He completed his residency in family medicine at Mercy Health System in Janesville, Wisconsin.

“I want my patients to walk away from each visit feeling that they have been thoroughly listened to and cared for beyond measure,” said Dr. Forte.

When not in the office, Dr. Forte enjoys fishing, traveling, spending time with family and friends, working out, running, mountain biking, camping, football and other sports. He also enjoys watching dramatic, action and adventure television shows.
