Sarah Eskofski, APNP, joins Affinity Medical Group

(Menasha, Wis.) — Affinity Health System and Ministry Health Care are pleased to welcome Sarah Eskofski, APNP, to Affinity Medical Group’s Employer Solutions department. She will be providing care at 1186 Appleton Road in Menasha.
An advanced nurse practitioner certified in occupational health and wound care, Eskofski provides medical treatment to people of all ages. She has a special interest in patient education related to disease prevention and health management. She plans to obtain Department of Transportation (DOT) certification in the near future.
Eskofski holds a master’s degree from Concordia University in Mequon, Wisconsin and a bachelor’s degree from Marian University in Fond du Lac, Wisconsin. She is also a certified family nurse practitioner (FNP) and a certified wound care nurse practitioner (CWCN-AP).
"My goal is to work collaboratively with patients and families to provide evidence-based medical care and prevent disease," said Eskofski.
When not in the office, Eskofski enjoys spending time with her family and friends, cooking and gardening.
