Submitting a Concern to JCAHO

Affinity Health System is committed to patient rights and organizational ethics.

In support of these commitments and goals, Affinity Health System shall provide a system of complaint and grievance management in order for patients and visitors to address problems and concerns about their care, treatment and interactions, privacy-related issues, and patient safety concerns with Affinity Health System and staff.

Patients receiving care within Affinity Health System care venues have a right to voice their concerns about service provided. An online form for submitting concerns/complaints or praise is available at http://www.affinityhealth.org/OffNav/Contact-Us.htm.  Appropriate staff personnel will address all concerns/complaints. If immediate staff cannot resolve an issue, employees or patients are encouraged to contact the facility administration by telephone and or mail so that all complaints can be evaluated, investigated and equitably resolved.

If the concern cannot be resolved at this level, individuals are encouraged to contact the Joint Commission on Accreditation of Healthcare Organizations at:
