Diabetes and other nutrition and endocrine-related disorders can seem like a roadblock to a “normal” life, but the staff at Affinity Health System helps you learn how to care for yourself properly. We help you regain as much freedom and control over your health as possible.
Affinity offers medical care and self-management training for diabetes at various locations throughout the Fox Valley.
Team of Specialists
The Affinity Diabetes, Nutrition and Endocrine Center brings together a team of health care specialists including endocrinologists, advanced practice nurses, certified diabetes educators and registered dietitians, office nurses and receptionists/schedulers.
As specialists we can find the hidden cause of your symptoms, obtain pertinent labs and treat you effectively with focused one-on-one care.
Our specialists work closely with your primary care doctor and with doctors who specialize in other areas affected by diabetes to make sure you are getting consistent quality care.
The care providers at the Affinity Diabetes, Nutrition and Endocrine Center work with the pediatric endocrinologists at the Affinity/UW Children’s Health Center in Appleton to care for children with type 1 diabetes.

Tests and Treatment
Our team of specialists will help you determine appropriate tests and technology to support you in your efforts to manage your blood sugar safely.
To help our patients with diabetes learn how to best control their illness, we offer group and one-on-one teaching. Each month, classes on lipid management and pre-diabetes management are offered. The Affinity Diabetes, Nutrition and Endocrine Center also offers cmmunity presentations on diabetes as requested.
Successful treatment of diabetes requires the patient’s full-time cooperation. We understand that learning how to take care of yourself after you’re diagnosed can feel overwhelming. We offer training to help you self-manage your illness and ease your concerns about living with diabetes.
Training topics include:
blood glucose (sugar) monitoring and treatment
diabetes medications: oral and insulin
benefits of exercise
guidelines for healthy eating and dining out
meal plan and medication adjustments during illness
how to prevent or delay complications
