Your health is multifaceted. Your care team should be, too. At Affinity Health System, we’re pioneering a new way of caring for you, covering all aspects of your health and well-being. It’s called the Affinity Medical Home.
Your personal care team
The Affinity Medical Home is a team approach, uniting doctors, nurses and specialized staff as your personal allies for primary care. These highly skilled professionals get to know you, understanding every aspect of your health. They work together to treat your whole person – body, mind and spirit.
Each time you enter the doctor’s office, you’ll meet with the same team, allowing you to build strong relationships with people who take a personal interest in your health. Depending on your unique needs, your team members may include:
- your doctor
- a nurse practitioner or physician’s assistant
- a social worker or counselor
- a nurse specialist trained in preventing and managing chronic conditions
- health care associates
- patient service representatives.
When further care is called for, all teams have access to Affinity’s network of specialists and our three hospitals, providing advanced care and state-of-the-art technology for all your health needs.
Revolutionary care for women
Imagine your family doctor and your obstetrician working side by side for your benefit. It’s happening now at the Affinity Medical Home, where we’re pioneering a total approach to primary care for women. Your obstetrician/gynecologist collaborates with your primary care doctor and other team members to provide the full spectrum of care for women through childbearing years and beyond.
