Unable to make decisions about your health care? Would your doctor or family know your wishes? An advance directive can state in writing your choices for treatment based on personal values and beliefs. Trained and experienced "Respecting Choices Facilitators" will provide instruction, answer questions, and provide group or individual assistance with completing advance directives for health care. Free.
