St. Elizabeth Hospital Community Foundation
Founded in 1976, St. Elizabeth Hospital Community Foundation serves as the philanthropic arm of St. Elizabeth Hospital, Calumet Medical Center and Affinity Medical Group clinics located throughout Outagamie, Calumet and northern Winnebago Counties. The foundation’s primary purpose is raising and distributing funds for equipment, projects and programs in Affinity’s northern service region, with an emphasis on serving the poor. The foundation also oversees a community endowment fund, awarding grants to area non-profit agencies. (However, no donations are directed to this fund.) All gifts to the foundation benefit one of Affinity’s entities, as directed by the donor. Donors are recognized through the foundation’s annual report and on donor walls, as well as at an annual appreciation event. The mission of the St. Elizabeth Hospital Community Foundation is to live out the healing ministry of Christ with a special emphasis on those in need, by generating, stewarding and distributing funds to enhance the quality of Affinity Health System’s services and programs and other community health care initiatives.

Helping the Poor
As a Catholic health care system, Affinity knows the needs of our community, and we recognize the dignity of each individual. The foundation takes a special interest in assisting underserved patients and their families.
Throughout our communities, there are caring, hard-working people who find themselves in difficult, unpredictable circumstances. The St. Elizabeth Hospital Community Foundation is there to provide assistance. Working through Affinity’s patient financial advocate, we provide funds to those who have nowhere else to turn.
