Community Sponsorship Guidelines

Affinity Health System is committed to supporting local organizations and charitable events aligned with our mission of providing services that promote the health and well-being of the communities we serve.

As a healthcare system that serves many communities, Affinity Health System is pleased to consider requests in support of nonprofit organizations. Each year we receive hundreds of donation requests, and unfortunately we are not able to fund every project or event. In many cases this is not a reflection of a particular project or organization, but the reality of allocating limited resources towards the many requests we receive.

Sponsorship requirements

To be considered for contribution, the request must:

  • Support Affinity Health System Mission, Vision and Values.
  • Benefit the community we serve.
  • Benefit a 501(c) (3) non-profit organization.

Items not eligible for funding:

  • Annual Campaign Support
  • Capital or Construction Projects
  • Program Funding Requests
  • State or national funding requests
  • Benefit events whose primary beneficiary is an individual or family

Request application process:

Sponsorship requests submitted through this online application are regularly reviewed. To allow sufficient time for processing and review, we recommend that sponsorship requests be submitted at least 3 months before an event or donation deadline.

Sponsorship requests may be made by completing the online application. Please note that this application has a 20-minute window for submission completion. Before starting your sponsorship request, we recommend that you gather all information needed to complete the sponsorship request application.
